Clay County Utility Authority customers:
Please scan and email (dolphinbf@gmail.com) or fax (904-215-6025) your notification and BPA Field test report (if included) from CCUA.
Town of Orange Park, City of Green Cove Springs,
St. Johns County, Beaches, and other customers:
Please email (dolphinbf@gmail.com) or fax (904-215-6025) your single page notification from the utility company to us.
We will process all of the paperwork and provide you with a copy of your test report! We submit the original report to the municipality.
